The Leadership and Management Processes (LAMP) Questionnaire is a self-report and 360-degree questionnaire designed to provide you data and information about your leadership and management style and approach. The questionnaire provides you valuable feedback that you can act on to build your capacity in both leadership and management.

The questionnaire consists of two parts, Part A and Part B. You are asked to complete both parts and also invite additional people (observers) to complete the questionnaire. This will form the 360-degree feedback component. Please choose at least you immediate supervisor or manager, two peers and two subordinates. It would be most useful if you could also choose an internal or external client.

Please provide the details on the observers you wish to complete the questionnaire in the About You section below.

The LAMP Questionnaire takes between 10-15 minutes to complete. Importantly, when completing considering the keywords or behaviours, do not spend too much time thinking about the meaning or intent of the keyword or behaviour. Try and select the response that comes to you first.

Once you have completed the questionnaire, click the submit button and your responses will be sent to George Siokos Consulting. Once all feedback has been collected a confidential summary report will be provided to you by George Siokos Consulting within 5 working days that includes feedback from your observers.

Part A: LAMP Behaviours Questionnaire:

The LAMP Behaviours Questionnaire contains 24 behaviours that reflect styles typically demonstrated by leaders and managers. Review each behaviour and click the button in the column that best describes the extent to which you demonstrate that behaviour. For example, for “I see change as an opportunity” if you believe that is true for you most of the time then click the button in the “Mostly” column.

Part B: LAMP Keywords Questionnaire:

The LAMP Keywords Questionnaire contains 106 keywords that describe styles typically demonstrated by leaders and managers. Review each keyword and click the button in the column that best describes the keyword in relation to your style. For example, for “Opportunistic” if you believe the keyword describes you some of the time, click the button in the “Sometimes” column.
